Grade requests

The Grade Release form grants the Dean of Students Office the permissions to pull members' GPAs to be submitted to their national organization for membership requirements. 

Grade Release Form

1. Download the form.
2. Complete the form, including all member's information.
3. Print the form.
4. All members must sign the form next to their name.
5. Submit the form to the Dean of Students Office at doso@wayne.edu or 301 Student Center.

A staff member will process the document and prepare it to be picked up by the requesting party.

The Dean of Students Office will not send any notice or documentation about grades via email to any student, organization or national organization. The student requesting the grade verification will be responsible for its dissemination.
